Problem: Ford Motor Company (Ford) is recalling certain 2015-2019 Flex, 2015 Explorer, Lincoln MKT, Lincoln MKZ, 2015-2016 C-Max, Escape, Taurus, 2016 Fusion, 2018-2019 Taurus, 2019 Lincoln MKT, Fiesta, and 2020 Mustang vehicles. The rearview camera may display a distorted, intermittent, or blank image when the vehicle is in reverse.

Risk: A rearview camera that fails to properly display an image can reduce the driver’s view behind the vehicle, increasing the risk of a crash.

Fix: Dealers will inspect and replace the rearview camera as necessary, free of charge. This will be a phased campaign, with the remedy becoming available in different phases based on model and model years. Interim letters notifying owners of the safety risk were mailed on October 25, 2025. Additional letters will be mailed once the final remedy is available on each phase. A software error may cause the rearview camera to display a blank image, or the image may remain on the display after the backing event has ended.

Risk: A rearview camera that displays a blank image can reduce the driver's view behind the vehicle. A rearview image that continues to be displayed after the vehicle is shifted out of reverse may distract the driver. Both of these situations can increase the risk of a crash.

Problem: Ford Motor Company (Ford) is recalling certain 2020-2023 Mustang vehicles. The brake fluid level sensor may not activate the visual warning indicator when the brake fluid is low. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 135, "Light Vehicle Brake Systems."

Risk: Without a low brake fluid warning light, the vehicle may be driven with low brake fluid, reducing braking performance and increasing the risk of a crash.

Problem: Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2020-2021 Avalon, Avalon Hybrid, Corolla, Highlander, Highlander Hybrid, RAV4, RAV4 Hybrid, Lexus ES350, Lexus RX350, Lexus RX450H, 2021 Sienna Hybrid, Lexus ES250, 2020-2022 Camry, Camry Hybrid, and ES300H vehicles. A short circuit may develop in the Occupant Classification System (OCS) sensor, preventing the front passenger air bag from deploying.

Risk: An air bag that does not deploy during a crash increases the risk of injury.

Fix: Dealers will inspect and replace the sensor as necessary, free of charge. Interim letters notifying owners of the safety risk were mailed in February 2024. Problem: Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2020 Corolla Hatchback vehicles. A rear hatch wire harness connector may have been damaged during assembly, which may cause the back-up lights to fail.

Risk: If the back-up lights fail, there can be an increased risk of a crash while the vehicle is reversing.
